‘It took seven years to find out why our daughter died’

The NHS is missing opportunities to learn from patient deaths, according to a review by the Care Quality Commission.

The CQC says too many families are not being included or listened to when an investigation happens.

The government commissioned the review after a BBC investigation into Southern Health Trust.
